Application for Free School Meals

Please note that this is an on-line form and is NOT to be printed and filled in by hand.

Important Note before you begin your application
   
  Applications for Free School Meals must be made to the Local Authority in which the School your child attends.  
   

Parent / Carer Details
  Please ensure your details are correct before submitting the application. You must ensure that you provide us with your date of birth and National Insurance Number (or National Asylum Support Service reference number if applicable) in order for us to be able to assess your entitlement. Failure to provide this information will result in your application being returned.  
   

Pupil Details
  You will only be required to complete ONE application form per family. If you have more than one child for whom you wish to receive Free School Meals, please ensure that all of their details are inputted on the application form. Please note that the children for whom you wish to claim must be in attendance at an LEA maintained School. Children who are in part time attendance at a nursery are not eligible for Free School Meals.  
   

Who Qualifies?
  Pupils will qualify if parents / carers are in receipt of one of the following benefits:
  • Income Support.
  • Income Based Jobseekers Allowance.
  • Support under Part VI of the Immigration and Asylum Act 1999.
  • Income-related Employment and Support Allowance.
  • Child Tax Credit, provided they are not entitled to Working Tax Credit and have an annual income, as assessed by HM Revenue & Customs (HMRC) that does not exceed 16,190.
  • Guarantee element of State Pension Credit.
  • Working Tax Credit 'run-on' - the payment someone may receive for a further four weeks after they stop qualifying for Working Tax Credit.
 
  Young people who receive Income Support or Income Based Job Seekers Allowance in their own right are also entitled to receive Free School Meals.  
   
  All children in attendance at a Monmouthshire School who are Looked After by Monmouthshire Social Services will be entitled to a Free School Meal.  
   

Confirmation of Benefit Entitlement
  Parents / Carers will no longer be required to provide supporting documentation to verify their benefit entitlement. All we need is for you as the parent / carer to complete ALL sections of the application to verify your benefit entitlement with the Department for Children, Schools and Families (DCSF) database, a system supported by the Welsh Government.  
   

Year 7 Clothing Grant / Leisure Facilities
  If your child is eligible for Free School Meals and is in year 7 they may be entitled to a Welsh Government (WG) clothing grant, your entitlement will be assessed when you complete this application form. Please ensure that your bank details are inputted on the application form as payment for this grant will be made directly into your account. If you would like further information you are advised to contact the School and Student Access Unit on: - 01633 644508.  
  Please note that the grant is only available to residents that reside within Wales, as prescribed by Welsh Government Regulations.  
   
  Those pupils who are entitled for Free School Meals will also be eligible for free use of swimming pools and some other leisure activities within Monmouthshire Leisure Centres. Your details will be shared with the Leisure Services team who will automatically activate the Free Leisure pass following approval of your child's Free School Meal entitlement.   If you do not wish for your details to be shared with the above departments, please ensure you tick the appropriate box on the application form.  
   

Parental Declaration
  Please ensure that you accept the terms and conditions on the declaration section of the form. Failure to do so will result in us being unable to process your claim.  
   

PLEASE NOTE THE FOLLOWING:
  Your child will not be awarded Free School Meals until an application has been completed and authorised by the School and Student Access Unit.  
   
  The School and Student Access Unit will aim to process your claim within 5 working days (or in readiness for the start of the academic year), assuming that the application has been completed correctly .  
   
  It is your responsibility to notify the School and Student Access Unit on 01633 644508 to inform them of any changes to financial / family circumstances which may effect your entitlement. If you fail to notify the authority, you will be charged the full cost of any meals taken by your child(ren) from the date your benefit stopped.

The School and Student Access Unit will automatically reassess Free School Meal entitlement on a monthly basis. Authorised claims will be ended and Free School meals stopped if the reassessment process identifies that a claimant no longer meets the eligibility criteria. Claimants will be informed if the entitlement is to be stopped and asked to provide evidence of eligible benefit entitlement prior to the claim being reinstated
